The rated voltage of the optocoupler is too low, which can result in the premature failure of the optocoupler.

• Choose an optocoupler with a greater breakdown voltage. For TOPSwitch designs, the breakdown voltage of the optocoupler should be greater than the bias winding voltage plus sufficient margin.
