Gain Limiting resistor RF3 is too large. TOPSwitch with Secondary TL431 Feedback requires at least 6 mA into the control pin for proper operation

• Decrease the value of resistor RF3.  

• To reduce gain further, try using larger values of RF1 and RF2.  

• Alternately, gain can be reduced by reducing the value of capacitor CF1
